Smile Enhancement - Advanced Cosmetic Dentistry

Drs. Glick and Mastrostefano believe that it is important that the patient be able to see how she/he could look with a new smile. This is accomplished through digital imaging. A digital "before" photo is taken of the patient, which is then sent to a lab for imaging. The lab digitally places an ideal smile selected by the patient over the patient's existing teeth. This dental imaging offers the patient the opportunity beforehand of being able to see how dentistry can change a smile and life.

Consultation and Payment

Four our larger case, a no-charge consultation appointment is scheduled in order for Drs. Glick and Mastrostefano to thoroughly explain treatment. Diagnostic models, digital x-rays, digital photos and a step-by-step treatment plan are all presented to the patient to help understand the procedures necessary to restore the patient to proper dental health.

Our office accepts MasterCard, Visa, American Express, Discover, cash, and personal checks as payment for dental care. We also participate in Delta Dental and Blue Cross Dental Insurance Plans. Payment is due at the time of service. Below is a copy of our patient consent form sheet, which all our patients must sign before receiving treatment.

  1. I hereby authorize payment of my dental and medical insurance benefits to Advanced Cosmetic Dentistry. I understand I am financially responsible for any charges whether or not paid by my insurance company, including those procedures deemed cosmetic or investigational.
  2. If co-payments and/or deductibles are designated by my dental insurance company or health plan, I agree to pay them to Advanced Cosmetic Dentistry at the time of service.
  3. I authorize Advanced Cosmetic Dentistry to release any information required to process and all claims for reimbursement on my behalf. A copy of this authorization may be used in place of the original.
Our office also offers financing through Capitol One, which includes loan programs with no interest for one year if the patient qualifies.
Porcelain Restoration Warranty

    All repairs or replacement of porcelain crowns, porcelain veneers, and porcelain onlays are warranted against breakage of five years, with the following conditions:

    1. The patient has followed all of the treatment and hygiene recommendations for the entire mouth as presented by Drs. Glick and Mastrostefano, including an occlusal mouth guard.
    2. Annual comprehensive oral examination by Dr. Glick and Dr. Mastrostefano.
    3. The restorations place by Dr. Mastrostefano must not have been damaged or lost due to any additional dental treatment, trauma, and/or periodontal disease.
    4. There can be no outstanding balance on account.
    5. The patient must have at least, two hygiene/recall visit per calendar year.
    6. Dr. Mastrostefano reserves the right to replace any restoration with a different material and/or procedures that could be more beneficial to the patient's health.
